Terms and conditions

 The therapist is required to act in accordance with legal regulations. The rights and obligations of both client and healthcare provider are, among others, laid down in the Medical Treatment Contracts Act (WGBO) and the Healthcare Quality, Complaints and Disputes Act (Wkkgz).

The therapist keeps a client file. The client has the right to access their own file. The file will be kept for a total of 20 years, in accordance with the legal retention period.

The therapist is obliged to provide the client with information regarding the treatment at all stages of the process. The client is obliged to provide relevant information to the therapist during the intake (anamnesis) interview.

The therapist observes confidentiality regarding all information provided by the client and stored in the file, with the exception of data for which the client has stated that they have no objection to its use for statistical purposes.

The therapist may not carry out any specific treatments or actions without the client’s consent.

The therapist is obliged to refer the client to another therapist or a physician if her treatment is not appropriate and/or sufficient.

Termination of the treatment may take place at any time by mutual consent. If the client no longer wishes or considers it necessary to continue the agreement, they may terminate it unilaterally. If the client terminates the agreement against the advice of the therapist, the client may be asked to sign a statement confirming that they have ended the examination or treatment prematurely against the therapist’s advice.

The therapist may only terminate the agreement unilaterally, stating her reasons, if it can reasonably not be expected of her to continue the agreement. In such a situation, the therapist will continue to provide support and advice until the client has been able to enter into an agreement with another healthcare provider.

The client agrees to the fees as stated on the website www.elmotion.nl.

Payment for treatments is made in cash per session or by bank transfer afterwards via invoice (possibly including a payment request link).

The client is obliged to cancel an appointment in time (at least 24 hours in advance). Otherwise, the costs of the reserved time may be charged.

For complaints about the treatment, the client can contact CAT for the complaints procedure.

Any adverse consequences resulting from withholding information present in the medical file of the general practitioner are the responsibility and liability of the client.
